A Wikipedia article about Napoleon, connected to maps. As you read, almost every place mentioned appears directly on a map. - Herodotus with maps and footnotes (Volume 1)
A book I downloaded from Project Gutenberg. I placed the footnotes into a separate document and added maps to make navigation easier. - Famous Men of the Middle Ages.

An older list of documents showing different ways visible connections can work. Some are unfinished and not especially useful on their own, but they’re good as demonstrations.
